Patient Bill of Rights: National Institutes of Health
 

  • To safe, considerate and respectful care, provided in a manner consistent with your beliefs;

  • To expect that all communications and records pertaining to your care will be treated as confidential to the extent permitted by law;

  • To know the physician responsible for coordinating your care at the Clinical Center;

  • To receive complete information about diagnosis, treatment, and prognosis from the physician, in terms that are easily understood. If it is medically inadvisable to give such information to you, it will be given to a legally authorized representative;

  • To receive information necessary for you to give informed consent prior to any procedure or treatment, including a description of the procedure or treatment, any potential risks or benefits, the probable duration of any incapacitation, and any alternatives. Exceptions will be made in the case of an emergency;

  • To receive routine services when hospitalized at the Clinical Center in connection with your protocol. Complicating chronic conditions will be noted, reported to you, and treated as necessary without the assumption of long-term responsibility for their management;

  • To know in advance what appointment times and physicians are available and where to go for continuity of care provided by the Clinical Center;

  • To receive appropriate assessment of ,and treatment for, pain;

  • To refuse to participate in research, to refuse treatment to the extent permitted by law, and to be informed of the medical consequences of these actions, including possible dismissal from the study and discharge from the Clinical Center. If discharge would jeopardize your health, you have the right to remain under Clinical Center care until discharge or transfer is medically advisable;

  • To be transferred to another facility when your participation in the Clinical Center study is terminated;

  • To expect that a medical summary from the Clinical Center will be sent to your referring physician;

  • To designate additional physicians or organizations at any time to receive medical updates.

Patient Expectations

We have a zero-tolerance policy for (actual or perceived) threats of violence or harm towards people or property. This may result in immediate termination of care.

We do not tolerate manipulative or aggressive behaviors.

Although we understand that what you are experiencing may possibly very difficult, we expect our clients to be polite and considerate. 

Patient Privacy/Confidentiality: Health & Human Services

  • The HIPAA Privacy Rule protects the protected health information (PHI) and medical records of individuals, with limits and conditions on the various uses and disclosures that can and cannot be made without patient authorization. This rule also gives every patient the right to inspect and obtain a copy of their records and request corrections to their file. There are specific forms that coincide with this rule: Request of Access to Protected Health Information (PHI); Notice of Privacy Practices (NPP) Form; Request for Accounting Disclosures Form; Request for Restriction of Patient HealthCare Information; Authorization for Use or Disclosure Form; and the Privacy Complaint Form.

  • The HIPAA Security Rule ensures the confidentiality, integrity, and availability of all ePHI.  Our clinic will protect against reasonably anticipated threats to the security or integrity of the information, protect against reasonably anticipated, impermissible uses or disclosures, and ensure workforce compliance.

  • To ensure patient privacy and confidentiality our clinic will not communicate with patients through social media platforms, such as Instagram, Facebook Messenger, or X (Twitter).

Payment Policy

Services are expected to be paid in full at the time of appointment. Appointments may be cancelled or rescheduled 24 hours before the appointment without penalty. Charges for appointments cancelled less than 24 hours will be subject to a $50 fee. We do ask for you to provide your credit card information at the time of scheduling for a pre-authorized payment but will not charge it until your scheduled appointment. For your convenience, we accept major credit and debit cards. Harmony Center does not accept insurance, but if you have an HSA or FSA account you may request reimbursement from your insurance provider.  If you are struggling to pay your bill, please discuss this with the clinic staff as a payment arrangement might be possible. If your account becomes delinquent, we may refer you to a collection agency.

Cancellation/Rescheduling

 

To be considerate of all our patients, if you arrive more than 10 minutes late for your appointment, you may be asked to reschedule or be seen for the remainder of your appointment time, but at full cost. If you miss or do not show up for your appointment, we reserve the right to charge your credit card on file for the full amount of the visit. Cancellation of appointments must be made 24 hours prior to the appointment. 3 or more no-shows or missed appointments in a 12-month period may result in the termination of our professional relationship. There will be a $35 fee for appointments that are not cancelled within 24 hours.

Controlled Substances

 

Medications are prescribed based on symptoms and conditions and these will be sent electronically to the pharmacy of your choice. Please contact the clinic through the patient portal  if you are in need of a refill. Please allow 2 business days for refills to be addressed.  Depending on your specific symptoms, you may be prescribed a controlled substance such as Vyvanse, Adderall, or Ritalin for ADHD or Klonopin or Ativan for anxiety. We do not prescribe any opioid medications. Please know that we may not prescribe a medication just because it has been prescribed to you in the past.  Please know that way may not prescribe a medication just because it has been specifically requested. Likewise, your provider reserves the right to discontinue or taper these medications.  All controlled substance prescriptions are evaluated on a case-by-case basis, adhering to evidence-based practice protocols. At present, the DEA allows prescribing of controlled substances via telehealth.  If this changes, we will update our website.
